WAIVER OF LIABILITY

I understand that I am participating in yoga classes and workshops - in person and remotely (livestream and recorded) - offered by Urban Yoga Chicago (“UYC”) and HealedLovedFed ("HLF") during which I will receive information about yoga and wellness. I recognize that yoga involves physical movements as well as an opportunity for relaxation, re-education and relief of muscular tension. As is the case with any physical activity, the risk of injury, even serious or disabling, is always present and cannot be entirely eliminated. I am fully aware of the risks and hazards involved. If I experience any pain or discomfort, I will listen to my body, adjust the posture and ask for support from the teacher. I will breathe smoothly.

I understand that it is my responsibility to consult with a physician prior to and regarding my participation in yoga classes and workshops and that yoga is no substitute for medical attention, examination, diagnosis and treatment. I represent and warrant that I am physically fit and I have no medical condition which would prevent my full participation in yoga classes and workshops offered by UYC and HLF.

In consideration of being permitted to participate in yoga classes and workshops, I agree to assume full responsibility for any risks, injuries or damages, known or unknown, which I might incur as a result of participating in the program. I understand that yoga practice and/ or specific poses are not recommended for individuals with certain conditions (i.e. cardiac illness, pregnancy, post-surgery). I affirm that I alone am responsible to decide whether to practice yoga and therefore assume the risk of yoga practice and release UYC, HLF, and the teacher(s) employed or subcontracted by UYC and HLF from any liability claims. 

In further consideration of being permitted to participate in yoga classes and workshops offered by UYC and HLF, I knowingly, voluntarily and expressly waive any claim I may have against Urban Yoga Chicago, HealedLovedFed, Joyce Doppelt (Savitree Kaur), Tijen Dyson (Jodh Kaur), and any instructors and wellness professionals employed or subcontracted by UYC and HLF for injury or damages that I may sustain as a result of participating in the program. 

I, my heirs or legal representatives forever release, waive, discharge and covenant not to sue for any injury or death caused by their negligence or other acts.

I have read and fully understand its contents, and I voluntarily agree to the terms and conditions stated above.
